The following are correct.

As an example...
An example of this is...
For instance ...

the following is wrong and irritates me no end when used.
"As a for instance"

Yeh, that always riles me up.

Also, people not understanding that "moot" can mean two things - if I "moot" something it doesn't mean "veto".
